APPLICANT'S STATEMENT & AGREEMENT

1. I understand that the Company may require me to submit to a test for the presence of drugs in my system prior to employment and at any time during my employment, to the extent permitted by law. I also understand that any offer of employment may be contingent upon the passing of a physical examination. I understand that bonding may be a condition of hire. If it is, I will be so advised either before or after hiring and a bond application will have to be completed.

2. I hereby certify that all the information that I have provided on this application is true and accurate to the best of my knowledge. I understand that any misrepresentation, falsification, or material omission of information on this application may result in my failure to receive an offer or, if I am hired, my immediate dismissal from employment. I authorize the Company the right to contact and obtain information from all references, employers, and educational institutions in order to verify the accuracy of the information contained in this application. I also authorize the references, employers, and educational institutions whom the Company contacts, to verify the accuracy of the information contained in this application. Further, I hereby release from liability the Company and its representatives for seeking, gathering and using such information and all other persons, corporations or organizations for furnishing some information, except as precluded by applicable law.

I understand that this constitutes an integrated agreement with respect to the at-will nature of my employment relationship, that is final and fully binding, and that there are no oral, written, or collateral agreements regarding this issue.

6. If any term or provision, or portion of this Agreement is declared void or unenforceable it shall be severed and the remainder of this Agreement shall be enforceable.

7.Any modification of this Agreement must be in writing and signed by you (or your representative) and the HR Director for the Company.
